UIC Reporting Forms (7520s)


The Underground Injection Control regulations require states to perform certain duties as part of the approval of their programs.  These requirements have been documented in the forms shown to the right.  The Office of Management and Budget has approved the forms for use nation-wide.  Each form has an estimate of the burden to complete it.  The forms are in Adobe PDF format, so you need the Acrobat Reader to use them.   Click on the individual form to view or download it, depending on your browser.  You may want to right click each link to determine the options you have.  This election is platform dependent.

You can request instructions on how to complete the forms by either calling Bob Smith at 202 260-559 or emailing (smith.robert-eu@epa.gov) him.  Make sure you specify your mailing address when calling or emailing.  We will post the instructions in this site once we digitize them.
