Please indicate the number of documents desired. The fee for documents is $20.00 for the 1^st copy; $10.00 for each additional copy requested at the same time. Check or Money Order must be signed, dated and made payable to Department of State. Remittance must be payable in U. S. dollars through a U.S. bank. Non-U.S. money is not acceptable. Please do not send cash.
